Replacing Certificates/Cards
Certificates may be replaced for $13, while membership cards may be replaced for $5.
Who pays for the cost of replacement is determined as follows:
- If the member has lost the certificate/membership card, then the member is responsible for the payment.
- If the local chapter made an error in the spelling of the name, then the chapter is responsible for the cost of the replacement.
- If the printer made an error that can be shown by our records, then the certificate/membership card will be replaced at no cost to the member or the chapter.
For replacement of certificates/membership cards the registration form needs to be completed as per instructions on the registration form. Replacement certificate/membership card orders may also be included with a regular order.
